Il permet un dépistage néonatal. Il est effectué sur une goutte de sang séché, prélevée vers le troisième jour de vie. On dose la TIR, proenzyme sécrétée par le pancréas qui apparait dans la circulation sanguine. En cas de TIR élevée, il faut associer le test à une recherche des principales mutations du gène CF afin d'éliminer les faux positifs. En effet, si la sensibilité du test est de l'ordre de 100%, sa spécificité est moins élevée. On peut ainsi dépister la maladie des enfants ayant une mucoviscidose avant même les premiers symptômes.
